Get "out of sync" error when uploading to Moteino

I have 2 Moteinos. One is the usb version and the other is a regular version with a ftdi adapter. When uploading to the usb version, everything works great. However, when I try to upload to the one with the ftdi adapter, I get an error and the upload fails. The one that that fails is new and I have not been able to upload to it successfully.

When each board is plugged in, they come up as different comm ports. I am setting the ide port setting to the comm port that I see in device manager for that specific device.

Do you have a LowPowerLab FTDI adapter?
You say the upload fails, but does the upload start at all?
Is there any blinking on the FTDI adapter after clicking UPLOAD?
Do you have any other FTDI device that you could try so that we isolate the problem to the Moteino or the FTDI?
Also try a short shielded USB cable if possible.

The FDTI adapter is the lowpowerlab one. I have not changed the jumper settings.
The other device is the USB moteino.

I have 2 computers here. System 1 is a Windows 7 32 bit and system 2 is a Window 8.1 64 bit.
The usb version works on both computers with no problems.

On the windows 7 system, plugging in the board that does not work does not always create a new comm port. Even when a new comm port is created, I don't see the led on the FTDI adapter blink when trying to upload a program.

On the windows 8.1 system, plugging in the board that does not work gives me a "the last USB device that you plugged in has malfunctioned and is not recognized by the system". Just to make sure, I rebooted the system and tried it again after the fresh reboot. Same error.
Of course, it does not create a new comm port in device manager.

Has that FTDI adapter in question ever worked?
What revision is it?
Can you try another USB cable? It almost sounds like a faulty cable. All are tested here before dispatch, so usually very low chance there is a DOA item after shipping.

This is the first time that I have used the FDTI adapter. The adapter and Moteino was purchased from you on 2/12/15.  The FDTI adapter is R2 and the Moteino is R4.  I was using the same cable for the USB Moteino as well as the one that does not work.  I just tried a new cable and I have the same symptoms.

Ok, one last thing, can you check for any visible cold solder joints on the USB connector and on the board where it might be a broken connection or damaged component or anything like that?
Maybe an upclose photo posted here would help.
Otherwise I can offer a replacement if you return it.
