If the RFIO is shared the maximum output is 14dBm whereas if the RFIO are split into separate antennae the maximum is 20dBm.
So how are people reaching 20dBm ? Is this purely setting the output to 20dBm in software?

Again, maybe it would be helpful to define what you are asking about. Ie "How is 20dBm possible in SXxxxx / RFMxx". Can you perhaps update the title? Thanks.
